8.02     REPORT FORMS

Construction inspection personnel are responsible for field sampling and testing in accordance with requirements of Materials I.M. 204 and those outlined in Construction Manual Chapters 2 and 3.  Forms are supplied for reporting test results, submitting samples, and as inspector work sheets.

 

Under the certified plant inspector (CPI) program and Quality Management - Asphalt (QM-A) program, specific sampling and testing will be done by contractor's representative (as directed and witnessed by contracting authority) per Materials I.M. 213 and Construction Manual 3.07, 3.21, & 3.22.  Plant inspectors and other certified HMA technicians must also be familiar with applicable Materials I.M.'s.

 

Form Identification and Use

n  "Asphalt Paving Daily Plant Report" documents daily plant operation, job control testing, asphalt mixture property results, and material placement for asphalt mix production and placement.  This form is available within the “SHADES” program, and its use should be encouraged to reduce errors and promote more timely electronic distribution of results.  Copies should be sent to the Construction and Materials Bureau, the District Materials Office, the project engineer and the contractor.  A blank copy of the form is included in Appendix 8-3.

n  “Iowa DOT HMA Field Report” is an electronic form used by the inspector to gather daily HMA quantities, measurements, and other project information before it is entered into the Asphalt Paving Daily Plant Report.  It provides a single document that may be shared between the inspector, plant monitor and contractor’s CPI to exchange and verify information.  A blank copy of the form is included in Appendix 8-5

n  "Identification of Sample for Test" (Form 820193) must accompany all samples submitted to Central Materials Laboratory and District Materials Laboratories.

n  "Noncompliance Notice" (Form 830245) is used to report project test results, workmanship, or other items in violation of specifications.  Copies are to be given to project engineer and contractor.
