I need help with an Iowa DOT PDF form

Some of the pdf forms on our website are set up to be filled out online, which requires Adobe Reader. Chrome, Firefox, Edge, and Safari browsers, by default, have their own pdf readers enabled, even if you have Adobe Reader installed on your computer. If using those browsers, you will need to download the pdf file to your computer and view the form in Adobe Reader. To view or fill-out our PDFs, you MUST open the PDF file in Adobe Reader and NOT a web browser.

If you have problems opening your document, please follow these steps:

Save the PDF to your local hard drive
Check to see if you have Adobe Reader installed on your computer
Open your file in Adobe Reader

** If PDF is blank, you may need to enable all features in Adobe Reader to trust and view document.

Troubleshooting

If you have Adobe Reader installed and your PDF still won't open:

Right-click on the PDF file and select "Open With"
Select "Adobe Reader" from the list of programs
If Adobe Reader is not listed, check to make sure it is installed.
