Report an Issue Using the Contact Form

To report an issue on an Iowa roadway, such as potholes:

  1. Visit the Iowa DOT contact form.
  2. Choose the topic: "Report an issue on the roadway."
  3. Select whether it's a city, county, or state roadway.
  4. Complete the form. If it’s a city or county roadway, you may need to contact the appropriate local agency.

Report an Issue Using the 511 App

Report roadway issues directly to our 24/7  Traffic Management Center (TMC) using the 511 app. This option requires you to be at the location of the issue when reporting.

How to Report

Option 1: Using the Iowa 511 App

  1. Open the Iowa 511 app on your smartphone.
  2. Tap the  "Report a Concern" button - the blue exclamation mark in the upper right corner of the map.
  3. Select the type of issue: Traffic, Road Hazard, Flooding.
  4. The app will automatically log your location (you must be at the site of the issue).

Only report if you are stopped or a passenger—never while driving.

iPhone users can report hands-free using Siri:
Just say: “Hey Siri, report a concern to Iowa 511.”

Android users: Hands-free reporting is coming soon!

Option 2: Calling 511

  • Dial 511 from your phone and say "report an issue on the road" at the main menu.
  • Note: Location is not automatically logged when calling, so be ready to describe it clearly.

What You Can Report

  • Traffic issues (e.g., crashes, standstill traffic)
  • Road hazards (e.g., potholes, debris)
  • Flooding or water over the road

Note: Reports can only be submitted for Iowa DOT-managed roads (Interstates, U.S. highways, and State highways). The system will notify you if the location is not eligible.

What Happens Next?

Your report goes directly to the Traffic Management Center (TMC). TMC staff will:

  • Review your report.
  • If needed, contact the appropriate maintenance garage to respond.
