Any school district, area education agency, merged area school, other agency, or individual planning to offer a driver education course must receive course approval from the department prior to the beginning of the first class that is offered and annually thereafter. Course approval will be issued for a calendar year or remainder of a calendar year. The approval expires on December 31 and must be renewed annually (IAC 761-634.4(1)).

  1. Print and complete forms 431183 and 431181 (Due annually on Dec. 24th)
  2. Sign and date forms (by school superintendent or principal)
