Submit Your Request Electronically
- Complete the Application for Persons with Disabilities Parking Permit for Iowa Residents form.
- When prompted on the form, upload your medical statement or your US DVA certification (for veterans).
- After completing the form, electronically sign and click submit.
Go to a DMV Location
You can go to any Iowa DMV. No appointment is necessary; however, you will need to wait for the next available team member. If you prefer, you can schedule an appointment. Additionally you can also be served by your local county treasurer's office, however, please check with their office to see if you need an appointment.
Be sure to bring your completed Application for Persons with Disabilities Parking Permit for Iowa Residents and your medical statement or your US DVA certification (for veterans).

Motor Carrier Services (Semi Registration)
Appointments are only being accepted in our Motor Carrier's Office located in Ankeny for:
- International Registration Plan (IRP)
- International Fuel Tax Agreement (IFTA)
- Intrastate Travel Authority
- Prorate titles for commercial vehicles
- Oversize/Overweight permits
Select "Motor Carrier services (No DL/CDL Services)" branch from the drop down menu when scheduling an appointment.
